The Clubs Form is the all-in-one form to let Clubs Team know what your club needs. Let us know if your club needs: recognition, to schedule regular meetings, supplies, help with planning and/or funding an event or travel, to let Clubs Team know about any leadership changes, a locker, and more!

Open Event Planning & Food Request Packet

This packet is for planning your event, and includes the required prior approval form. Once completed and signed, Clubs Team will order the food for you. After you request your event by filling out the Clubs Form (above), a Clubs Consultant will reach out to you to schedule you for an Event Planning meeting so you can get one-on-one help completing this packet.
